Migrant Health Awards
 
Who, in the migrant health center arena, is setting new standards of patient care and quality to better meet the challenges of the future?
 
Each year, NACHC honors outstanding service to the Community Health Center Movement. The 2011 Migrant Health Awards will be presented during the NACHC National Farmworker Health Conference, May 11-13, 2011 in Delray, Florida. 

The awards span the spectrum of migrant health center service and we invite you to nominate migrant health centers, migrant health center board members and staff, as well as, public officials whose individual accomplishments best exemplify excellence in migrant health care and contribution to the health center mission.
 
NACHC will now be accepting nominations through Friday, February 18, 2011

Note: You must be an Individual Member of NACHC in order to submit a nomination. You also may submit more than one nomination. In a nomination, consider the following questions:
 
Individuals and organizations nominated must fully meet the specific award criteria in order to be considered. A complete nomination package contains:
·         The principal nomination form to be accompanied by a written nomination statement and;
·         Two additional supporting nomination forms. Should you need additional forms, please make copies or download the forms by clicking here.   
 
Remember, you must be an Individual Member of NACHC in order to submit a nomination(s). 

Payments for Individual dues may be included with your nomination. Annual Individual dues are $30 for Basic Membership and $65 for Full Membership (which includes a subscription to Community Health Forum magazine). Contact the NACHC Membership Department regarding membership details at 301.347.0400 or membership@nachc.com.
