
The FQHC Brand Identity Logo
As a visual symbol of who and what we are, our national brand logo represents the following major traits of all Community Health Centers (FQHCs):
· Their fees are based on the patient’s ability to pay
· They provide quality health care to all people regardless of insurance status or income level
· The are staffed by highly competent health care professional teams
· They are community controlled by patient majority boards of directors
The FQHC logo is not intended to supplant individual health center logos, but rather to supplement and unite those symbols nationwide. Just as important, the national logo is intended to distinguish FQHCs from non-FQHCs that may use the generic term “community health center” or “health center” in their corporate names.

Other entities interested in using the brand identity logo must obtain the written permission of the National Association of Community Health Centers. This may be requested in writing to: ycrenshaw@nachc.org.
The National Association of Community Health Centers, Inc. owns these certification marks in the U.S., which it licenses for use to organizations who meet its standards for use of the certification marks.
