The Quality Management Plan: A Practical, Patient-Centered Template

As health centers today play an increasingly important role in the nation’s health system, they face an expanding number of new initiatives and related calls for ever-greater accountability. A Quality Management (QM) Plan can help health centers seamlessly integrate new programs, care models, and advancing technologies with the primary care services they have long provided to improve the health of their patients and communities. The Quality Management Plan: A Practical, Patient-Centered Template, written by Dale S. Benson, MD, FACPE and Peyton G. Townes, Jr., MHSA for the National Association of Community Health Centers (NACHC), provides a basic blueprint for the QM Plan of a typical mid-sized health center, which can then easily be modified as needed to accommodate both smaller and larger, more complex organizations. Updated from 2009, this monograph focuses on organizing a QM program around the Patient Centered Medical Home (PCMH) model. It includes four program-specific “Metric Paks” for Primary Care, HIV, Behavioral Health, and Meaningful Use, which are structured around a quality framework that aligns with the building blocks of PCMH. |
